RETURNING ATHLETES:
Please download the following insurance form and send to the address below. You will fill out a returning athlete questionnaire during your returning athlete physicals for your clearance. If you have had a significant injury requiring medical attention during the summer, please notify your athletic trainer before your return to coordinate medical clearance. Please note your returning athlete physical date and time using the link above.

Please download the following forms. This information is utilized by the sports medicine department's athletic trainers and team physicians for your medical clearance and best possible coordination of your medical care while participating at JHU. To avoid unnecessary delays in your clearance and sport participation, please turn in your forms before the August deadline. If you have any questions concerning these forms or your clearance, please feel free to contact an athletic trainer. It is very important that the forms are accurate and complete. All JHU athlete medical records and personal information is kept completely confidential and stored in a secure record room.
