How can I be added to an animal protocol
or get access to an animal room?
- Take the animal care and use online training (https://secure.lwservers.net/default.cfm )
- Enroll in the Animal Exposure Surveillance Program (AESP) through Occupational Health Services.
Go to 98 N. Broadway (Church Home Bldg.) 4th floor and tell them you would like to enroll in the AESP and get the enrollment certificate before you leave.
- The PI needs to fill out and sign the change in personnel amendment request form (found on our website - www.jhu.edu/animalcare). Be sure to answer the four questions on the first page of the form.
- Obtain the card/badge access form appropriate to the building in which you need access and have the PI sign it.
- Fax the change in personnel amendment request form, a copy of the AESP enrollment certificate, and the card access form to 7-3747 (or hand deliver to Reed Hall B122).
- Take the signed card/badge access form to Ross 459 (or appropriate RAR office) for approval.
