Animal Exposure Surveillance Program (AESP)

1. All completed Animal Exposure and Surveillance Program (AESP) questionnaires  must be sent to the Occupational Health Clinic, Blalock 139, via hand delivery, fax 410-955-1617, or email ebibb@jhmi.edu.
2. Once the Occupational Health Nurse, Ms. Ellen Bibb, has reviewed the questionnaire, an AESP certificate will be sent to you.
3. Send a copy of your AESP certificate along with the protocol form or personnel amendment to the ACUC office (Reed Hall B122) via hand delivery, fax 443-287-3747, or email acuc@jhmi.edu.
